Caregiver NOC Shift Available

This position will offer you flexibility and take a more personalized 1:1 approach to member care. Nursing Students are welcome to apply!

Full-Time Monday-Friday 11 pm-7 am position immediately available.

Community Horizons is on the quest to connect local, experienced caregivers to a fellow community member who is hiring a private caregiver. This is a pet-friendly environment with cats and dogs present. It is located in a quiet/safe neighborhood in Allouez, WI.

Hours: Tuesdays 11 PM – 7 AM

Pay: $16.32/Hour

TASKS:

  • Provide hands-on personal and supportive care in the comfort of the member's home located in Allouez (just minutes from the hospitals):
  • hourly bed check
  • incontinence care, including ostomy and catheter care
  • ROM exercises
  • skin monitoring
  • transfers using the Hoyer lift
  • light housekeeping (as needed)
  • Must have some knowledge and understanding of how to use an app to clock in/out in addition to documenting in a MAR located on Google Drive.

THE IDEAL CANDIDATE:

  • Maintain a valid driver's license and reliable transportation.
  • Pass a WI Caregivers Background check
  • Be dependable, on time, and have a can do attitude!
  • Previous caregiving experience is required!

Who is Community Horizons?
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.

Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.

While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agenthandling payroll and employment paperwork.
